By signing this document, I acknowledge the inherent risks and voluntarily assume full responsibility for any injury, damage, or loss that may result from my participation. I hereby waive and release the business, its owners, and its staff from any and all liability, past, present, and future, relating to the services provided.

Client Agreement · Studio Policies · Liability Waiver (New York)

WAIVER & ASSUMPTION OF RISK

I am voluntarily participating in Pilates, yoga, dance, and fitness programs offered by RIA Pilates. I understand that participation involves physical exertion and inherent risks of injury.

I acknowledge and agree to the following:

The exercise program utilizes Pilates, strength training, mobility work, and conditioning techniques. During exercise there exists the potential for muscle tears, soreness, sprains, strains, fractures, abnormal blood pressure, fainting, heart disorders, cardiovascular stress, and other serious injuries, including death. I assume all such risks and accept full personal responsibility for any injuries or damages that may occur.

I understand that I have the right to choose which exercises to perform and may stop participation at any time.

​

I understand that I should obtain medical clearance from a physician prior to participation if I have any health concerns.

​

I hereby release and hold harmless RIA Pilates, its owners, instructors, employees, and representatives from any and all claims arising out of ordinary negligence related to my participation. This release does not apply to gross negligence or intentional misconduct.

STUDIO POLICIES

All reservations for classes and appointments are subject to a 24-hour cancellation policy without exception.

​

Cancellations made less than 24 hours before the scheduled session or failure to attend (no-show) will result in a full session charge (100%).

​

All purchases, packages, and memberships are non-refundable.

​

RIA Pilates is not responsible for lost, stolen, or damaged personal belongings.

​

Late arrivals may be denied entry if more than 10 minutes late for safety and class quality reasons.

​

Appropriate fitness attire and grip socks are required at all times.

​

Pregnant clients may participate only with medical clearance and instructor approval.

​

EMERGENCY CONSENT

In the event of a medical emergency, I authorize RIA Pilates to seek emergency medical treatment on my behalf.

I have read and understand this waiver and voluntarily agree to its terms.

MINOR PARTICIPANT

If the participant is under 18 years of age, a parent or legal guardian must sign this Agreement. The parent or legal guardian agrees to all terms stated above and assumes full responsibility for the minor participant.

SEVERABILITY

If any portion of this Agreement is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.

ENTIRE AGREEMENT

This document constitutes the entire agreement between the participant and RIA Pilates and supersedes any prior discussions or agreements.

This Agreement is effective as of the date of Signature indicated below.
